A geotechnical distress investigation was performed by Geohazards, Inc of Gainesville, Florida, as requested by State Farm Florida Insurance Company. It was determined a possible cause of distress is due to the formation of subsurface cavities.  Geohazards has recommended that thirty-seven injection pipes, using 600-1200 cubic yards of moderate to low slump cement grout be used for remediation.
